TIZANIDINE HYDROCHLORIDE capsules, 2 mg are light yellow to yellow colored granular powder filled in size "5" empty hard gelatin capsule having light blue opaque colored cap imprinted with "1111" in black ink and light blue opaque colored body and are supplied as follows: NDC 70710-1111-8 in bottles of 150 capsules with child-resistant closure NDC 70710-1111-5 in bottles of 500 capsules NDC 70710-1111-4 in unit-dose blister cartons of 100 (10 x 10) unit-dose capsules
